If you cannot keep your appointment, please call us as soon as possible. We appreciate
24 hours notice.
Our
office works by appointment to efficiently serve you. This addresses the value of your
time and assures that specialized personnel and equipment are available for your
particular needs.
Emergencies
An
Ophthalmologist is on-call 24 hours a day, every day of the year.
We accept emergency calls at all hours. For emergencies any time, call:

Fees, Payment,
and Insurance
Prior to your appointment, please check with your insurance company
about specific coverage or limitations that may apply to your office visit. Some companies
require that you see a specific doctor or may require a referral from your primary care
doctor before you see one of our ophthalmologists.
Please bring your insurance card to your appointment. If your
health coverage changes, please let us know as soon as possible.
We participate with many different health
care plans, and we accept Medicare assignment. However, we require proof of insurance
before we can file a claim on your behalf. Please bring your insurance card with you to
all of your visits.
If your insurance card is not available,
payment is expected at time of service. You will be asked to pay your estimated patient
share or co-payment at your visit. If you are unable to pay at that time, a $10.00 billing
charge will be added to your account. For your convenience, we accept cash, checks, Visa, Mastercard and Discover. You can also make a credit card payment over the phone.
We realize that health insurance and
managed care plans can be complex. We will do our best to help you understand your
particular insurance plan, but strongly suggest that you become familiar with your
specific insurance coverage to insure that you are receiving the maximum benefit.

Eye Center of Northern Colorado Payment Policy
Payment is expected at the time of service. We accept cash, check and
for your convenience, Visa MasterCard and Discover. If full payment
cannot be made at the time of service, we also offer the following:
90 days same as cash.
3 equal payments to be received monthly for non-elective procedures.
Patient balance must be over $200 to qualify for payment options.

CareCredit If a balance is over $1000 , our patients can choose to
utilize the financing options provided by CareCredit. Please ask for a
brochure or use the internet link on this page.
You are not obligated to
use CareCredit. You can arrange financing through any institution of
your choice.
Elective/Cosmetic Procedures:
Full payment is due prior to or at time
of service.
CareCredit may be used as outlined above.

fitting experience. Bilingual in Spanish would be a plus. If you have a
